If we suppose that during a craniotomy, the neurosurgeon happens to accidentally drop some lysergic near the surgical site, LSD doesn’t seem to have much trouble crossing the blood brain barrier. Though I’m not certain of its pharmacokinetics when administered parenterally, I imagine the patient would experience euphoria (assuming he’s awake). As I recall, LSD crosses even the integument very easily so I doubt it’d have much trouble crossing the exposed surgical field.
In short, the patient/subject would get high.

LSD also crosses most any membrane to enter the blood steam, such as skin, muscus, or probably the meninges on the outside of the brain, it also absorbed will from IM injections, so I expect the same would happen if it was “in” the brain, it then enter the blood steam, cross the blood brain barrier and produce the usual gamut of LSD effects.
